The film opens with an advertising salesman, Philip Woode, winning dinner for two on a TV show. He takes Danielle Breton, a French Canadian model who was part of the prank, as his date. The next morning, Philip is stabbed to death by Dominique, Danielle's twin sister, and dies trying to alert a neighbor. A reporter named Grace Collier calls the police, and Danielle's ex-husband Emil helps her hide Philip's body. Grace suspects that Danielle is hiding the murderer and hires a private investigator to gain access to the apartment. The investigator discovers the body and a file on the Blanchion Twins, Canada's first conjoined twins. Grace investigates further and learns that Dominique apparently died during their separation surgery. Emil and Danielle take Grace to a mental hospital, where Emil hypnotizes her and reveals that Danielle dissociates into a violent "Dominique" personality during sexual experiences. Emil is killed by Danielle when he tries to bring forth "Dominique." In the end, Danielle denies knowledge of the murders, and the sofa containing Philip's body is tracked to a remote train station in Canada.

Sisters debuted at Filmex in Los Angeles on November 18, 1972, and was later released by American International Pictures in the United States. It opened in Los Angeles on April 18, 1973, and later expanded to New York City on September 26, 1973, where it received positive reviews and played until November 9, 1973. The film was also showcased at the 1975 Venice Film Festival.
